In the last few years, the AMA has introduced several new codes intended to reimburse primary care physicians for managing patients with chronic illness, including transitions in care from inpatient to community-based settings and overseeing care for patient in home care. These have also been designed to provide better care as well as to reduce costs by reducing hospital readmission rates and emergency room visits. However, these codes have multiple requirements and documentation issues.
